General Admission Policy and Procedures I. Group Home Objectives It is the objective of David's House Ministries to provide an environment wherethe development of a resident's personal, social, recreational, emotional,developmental, and physical skills will be encouraged. This will beaccomplished through a Christ-centered family home atmosphere providing closesupport, access to community programs and services, and exposure to thedynamics of the community. II. General Admission Requirements - for all homes 1. A prospective resident will exercise free choicein electing whether to reside in the group home. 2. No individual will be denied admission based onrace, religion, color, sex, or national origin. 3. All prospective group home residents must beeighteen years of age or older at time of admission. 4. Applicants must be able to provide monthlymonetary requirements through RSDI, SSI, Network 180 (CMH), or private funds. 5. An applicant must have a diagnosis that fallswithin the scope of the license for the home in which he/she is anticipatingplacement. (See attached descriptions.) 6. An applicant will not be considered forplacement if he/she requires continuous nursing care. 7. An applicant will participate in and assist thelicensee in completing a written assessment of his/her personal care,supervision, and protection needs. This assessment is required for determiningthe kinds of services, skills, and physical accommodations that will berequired in the home for meeting the applicant's needs. This will also helpdetermine if an applicant is compatible with the other residents and members ofthe household. 8. An applicant must respond to instructionalcontrol of behavior and not require physical interventions, isolation, orrestraints. 9. An applicant must provide a health careappraisal that has been completed within a ninety-day period before theapplicant's admission into the home. 10. An applicant must display awareness andreasonable understanding of the house rules. 11. Prior to moving into a home, residents and theirparents and/or guardians must have resolved the question of limited or fullguardianship. This role will not be filled by DHM staff members or any boardmember. This does not prohibit a staff member or board member from serving asguardian for a family member. 12. An applicant must have self-care skills or bewilling to cooperate with personal care staff members to maintain a high levelof personal hygiene. 13. An applicant will be admitted only to the homethat provides the least restrictive setting that is appropriate for theindividual's care needs. This is determined by the intake committee incooperation with the applicant's family supports, placing agencies, doctors,etc. III. IntakeProcess: 1. All candidates for possible placement arereferred to the Director or to the Licensee Designee. 2. The applicant and/or the guardian or responsibleagency are contacted and provided with an Application Packet. 3. A tour of the home is scheduled. The applicantdoes not need to attend this tour if it is determined that it is in his/herbest interest not to attend. 4. The application packet is completed andsubmitted for review. The application must include a signed release forthird-party verification of the information. 5. After the packet is reviewed, a waiting listinterview is held. This meeting is intended to examine the application packetand gather any additional information needed to determine eligibility forplacement. The applicant is not required to attend this meeting. 6. DHM will obtain third-party verification, whichincludes receiving PCP and/or IEP documentation, as well as conductinginterviews with job coaches, teachers, supervisors, other family members, etc. 7. The applicant is either placed on the waitinglist or determined to be ineligible for placement. 8. A letter of acceptance (or of denial) to thewaiting list is sent to the applicant. This letter will give information aboutthe waiting list on which the applicant is being placed. If the applicant isnot accepted, he/she will be informed of the factors and reasons that led tothis decision. 9. The applicant remains on the waiting list untilthere is an opening or he/she is voluntarily removed. 10. When there is an opening, the intake committeedetermines which applicant on the waiting list to contact for an admissionsinterview. 11. If the applicant has not had a tour of the home,a tour is scheduled before the applicant has an admissions interview. Thisshould be on a day different from the interview day. 12. An admissions interview is held to provide anupdate of the applicant's current needs and to redetermine the suitability ofthe current program for meeting those needs. The applicant should be inattendance for this meeting. 13. The applicant is offered a sixty- to ninety-dayprobationary placement in the home. Before the placement begins, all of therequired resident information and documentation must be completed. Thisincludes both AFC requirements and HUD requirements. 14. If the sixty- to ninety-day probationary periodis successful, the applicant is accepted for permanent placement. House I Process: House I is different from the other homes in that itis a HUD Section 8 low-income project. The waiting list application asks onlyfor the information necessary to determine eligibility for participating in afederal housing assistance program. Once there is an opening, we contact thenext person on the waiting list and provide that individual with the fullapplication packet. This is followed by an admission interview, along withthird-party checks for eligibility determination. The process is fully outlinedin the Tenant Selection Plan for House I. Individuals on the House I waitinglist may request a copy of this plan.
